Dedicated to serving Washingtonians

Helping Washingtonians who rely on Medicaid live healthier lives

Community & State is the business segment of UnitedHealthcare that provides health care coverage for beneficiaries of Medicaid and other government health care programs. These health plans operate locally as the UnitedHealthcare Community Plan.

In Washington, our team strives to provide high-quality, affordable, and individualized care for our members, with a special focus on moms and families, rural communities, and groups experiencing health disparities. We support our local care and social support providers with tailored incentives and VBP programs to deliver the right care at the right time and place. The State of Washington’s goal is to provide whole-person care, integrating physical health and behavioral health services for better results and healthier residents, and we are proud to be a partner in their mission.

To continue to drive Washington Medicaid toward a best-in-class model we are:

  • Providing timely, integrated, whole person care delivery that drives quality population health outcomes
  • Developing innovative community-based, primary care, and FQHC partnerships with stakeholders that transform and simplify care
  • Utilizing a data driven, localized, coordinated approach that impacts health equity through social justice lens
  • Creating a Culture of Quality that lowers cost, increases member and provider satisfaction, improves outcomes, and impacts health equity (Quintruple aim)

UnitedHealthcare Community & State has in-depth experience and knowledge of the Washington market. We live here. We work here. Our strong local presence enables us to see the opportunities as well as the issues, so we can deliver effective solutions to address the specific needs of Washington.

It’s our privilege to work within the Washington healthcare system and the Washington State Health Care Authority to help people live healthier lives.

Addressing local population health outcomes through integrated care

Our person-centered approach starts by building a trusting relationship with individuals and their families. We work to empower our members to be active participants in their own health and aim to remove any barriers that stand in the way. Our highly coordinated services address physical and behavioral health needs, as well as social determinants of health such as food, clothing, shelter and transportation.
